Follow up - living in the community
- PACE’s outreach and follow up services help children who move on from PACE into mainstream and other special schools. Input is also offered to other children who may not have previously attended PACE.
- We believe families and school staff require an in-depth understanding of their child’s learning abilities and needs as well as the knowledge and skills to help him/her to become an active participant in their home and school life. Training on both a practical and theoretical basis is given.
- Current funding has enabled PACE to develop its support to schools and homes.
- The aims of our outreach and follow up services are to:
- Help children maintain and further develop their independence skills throughout their growing years
- Enable children to be supported across all areas of learning within other educational settings
- Provide a continuum of support to families so that children are as active as possible in their home and social lives

Therapists, conductors and teachers
- Help parents and local educational authorities to identify and choose a suitable school for a child to move into from PACE.
- Support the child and family through the transition.
- Work with the receiving school prior to the child’s entry. This can include training Learning Support Assistants in advance.
- Provide on-going support to the child once they are in their new school (for an agreed period of time).
- Assess options for the future in collaboration with the family and other professionals (continue with PACE input or hand over to NHS).
- Provide blocks of intensive input during out of school time at the PACE centre.
- Perform home visits and home training in order to support and develop each child’s independence at home.
- Run training courses at schools for teachers, LSAs, SENCOs and parents as well as periodic seminars for interested groups.
